Insurance Information

Elysian Renewal is a cash-for-service or an out-of-network provider practice. While we do not accept third-party insurance payments, we are an insurance-friendly practice. All payments must be made upfront, and we can provide you with an invoice or superbill to reimburse your insurance company. We have also partnered with Mentaya (we get *no financial benefit* from Mentaya)to help clients understand what their insurance will cover and take the hassle out of reimbursement. Alternatively, call your insurance company directly to confirm your out-of-network coverage.

Essential questions to ask your insurance provider are, “What are my out-of-network mental health benefits?” “What is my deductible or out-of-pocket max?” and “Do you cover session code 90834 for out-of-network benefits?” Elysian Renewal does not guarantee reimbursement from any insurance company.

We’ve partnered with Mentaya, a service that streamlines getting reimbursed for your therapy sessions through out-of-network benefits.

Mentaya is perfect if you:

+ Feel overwhelmed by superbills and insurance.

+ Have submitted superbills but failed to get any reimbursement.

+ Simply want to skip the hassle of paperwork!

Here’s how it works:

1. Sign up for Mentaya: https://mentaya.co/inviteclient/elysianrenewal

2. Our practice will enter your sessions into the platform.

3. Mentaya submits the claim and handles any insurance follow-up.

4. You get reimbursed by insurance!

Mentaya charges a 5% fee per claim, including handling any required paperwork, dealing with denials, and calling insurance companies.

Payments and Billing

All fees are required at the time of service. Elysian Renewal accepts all major credit cards, debit cards, Health Savings Accounts (HSA), and Flexible Spending Accounts (FSA). Please contact your HSA or FSA company regarding questions related to covered funds. A credit card on file is required to book an appointment. Please note that we do *not* accept cash.

Good Faith Estimate

Under Section 2799B-6 of the Public Health Service Act, healthcare providers and healthcare facilities are required to inform individuals who are not enrolled in a plan or coverage or a Federal healthcare program or not seeking a claim with their plan or coverage both orally and in writing of their ability, upon request or at the time of scheduling health care items and services, to receive a “Good Faith Estimate” (GFE) of expected charges. The GFE shows the costs of items and services that are reasonably expected for services provided by Elysian Renewal. The estimate is based on information known when the estimate was created. It does not consider any reimbursement you may receive due to out-of-network benefits you submitted directly.

The Good Faith Estimate does not include any unknown or unexpected costs that may arise during treatment. You could be charged more if complications or exceptional circumstances occur.

You can dispute the bill if you are billed for more than this Good Faith Estimate. For questions or more information about your right to a Good Faith Estimate, visit www.cms.gov/nosurprises

Elysian Renewal Boutique Therapy offers a variety of scheduling options. Our therapist primarily works through a HIPAA-compliant online videoconferencing platform. We are taking an interest list for in-office sessions and will open this option as soon as interest allows. Our counselor may have weekend, evening, and early morning appointments available for your convenience.

Cancellation Policy

Elysian Renewal offers reminder texts to notify you of your appointments. In the event you do not arrive for your scheduled therapy appointment and you have not informed the office at least 24 business hours (our office is closed on weekends and holidays) in advance to cancel or reschedule your appointment, you will be required to pay the full fee for the time of the appointment.
